Our 50-bed, Medicare- and Medicaid-certified skilled nursing and rehabilitation home offers short-term recovery services for patients working to regain strength and avoid re-hospitalization after a hospital stay. Daily, individualized treatment from a friendly, caring staff helps residents heal and get back to what matters most.

For residents who need ongoing support, Colonial Poplin offers long-term skilled nursing and rehabilitation care in a socially active community that feels like home - with individualized services designed around each resident's cognitive and physical needs.

Poplin Way is a socially active assisted living community with a charming, country feel. Residents live in their own apartment while having access to around-the-clock nursing assistance - plus the added peace of mind of skilled nursing care right on-site, if it's ever needed.

“This has been our first experience having a loved one in a rehab facility and we have been VERY impressed! The staff is friendly and obviously are great at their jobs. They listen to patients and their families and make every effort to help in any way they can. Dad got PT, OT, and speech therapy during his stay and could see him growing stronger day by day. The food there is exceptional as well, with a nice variety of choices with specific dietary needs in mind. Every day a special flyer is delivered with different activities to help residents pass their time. There are many rooms for residents and visitors to spread out and either socialize or have private time. We can't thank the staff at Colonial Poplin enough for the great care our dad has been receiving.”
