Founded by the Philbrick Family in 1983, our care community offers skilled nursing, rehab services, long-term, and respite care at Colonial Poplin, and a quaint assisted living at Poplin Way, set in a charming Colonial-style residence in Fremont, NH.

Kim Piper, NHA, RN
Executive Director of Colonial Poplin Skilled Nursing Community
Kim brings nearly two decades of experience in skilled nursing, having begun her healthcare career as a Licensed Nurse’s Aide and then obtaining her Registered Nurse license. Throughout her tenure at Colonial Poplin, she served as Director of Nursing Services, where she developed extensive expertise in all aspects of skilled nursing operations and resident care.
Now serving as Administrator of Colonial Poplin, Kim leads with an "all-hands-on-deck" approach, believing that every team member plays an essential role in providing exceptional care. She remains actively involved throughout the community, working alongside staff, supporting residents and families, and fostering a culture of collaboration, compassion, and excellence. Her commitment to hands-on leadership and quality care continues to strengthen Colonial Poplin's mission of serving residents with dignity, respect, and professionalism.

Alexz Pettengill, MNA
Administrator & Care Director of Poplin Way Assisted Living Community
Alexz is a dedicated leader and long-standing member of the Colonial Poplin and Poplin Way Communities. With extensive knowledge of assisted living services and a deep commitment to resident-centered care, she brings both experience and compassion to her role as Administrator and Care Director of Poplin Way Assisted Living.
Alexz leads with a collaborative, hands-on approach, working closely with residents, families, and team members to ensure a welcoming, supportive, and high-quality living environment. She is often found assisting across departments, reflecting her belief that exceptional care is achieved through teamwork and a willingness to step in wherever needed. Her approachable leadership, strong clinical knowledge, and unwavering dedication make her an invaluable resource to residents, families, and the entire Poplin Way community.

Reneè Conroy, RN
Director of Nursing Services at Colonial Poplin
Reneè brings more than twenty years of experience as a Registered Nurse with a passion for delivering exceptional care and an impeccable bedside manner. She also has extensive expertise in hospice care, having served for many years as a hospice nurse case manager throughout New Hampshire.
Renee is dedicated to providing therapeutic and compassionate care. She values open communication and works collaboratively with residents, families, physicians, and the interdisciplinary team to ensure each resident receives individualized, high-quality care.

Euckie Jembere, RN, ACWN
Assistant Director of Nursing Services and North Wing Short-term Stay Unit Manager at Colonial Poplin
Euckie has been a dedicated member of the Colonial Poplin team for many years, beginning her career in the nursing department as a Nurse’s Aide before advancing to Licensed Practical Nurse and ultimately earning her Registered Nurse credentials and joining our nursing management team.
Overseeing our short-term stay wing, she works closely with clients, their families, and responsible parties to provide compassionate care throughout their rehabilitation journey. She plays an important role in helping clients successfully transition from skilled nursing rehabilitation back to the community through collaboration, education, and thoughtful discharge planning. She also serves as our certified Wound Care Nurse, bringing specialized knowledge and clinical expertise to support positive outcomes for those under our care.

Sandi Moscillo
Admissions Director of Colonial Poplin
Sandi brings extensive experience and a deep connection to Poplin Communities, having been a long-standing care staff member of the Poplin Way team before transitioning into her role as Admissions Director for Colonial Poplin.
As Admissions Director, Sandi works closely with our regional clinical liaisons, referral partners, prospective resident/clients, and families to coordinate admissions for our skilled nursing, rehabilitation, and extended care services. Her knowledge of senior living and compassionate approach help ensure a smooth and welcoming transition into our community.
Known for her positive, jovial personality, Sandi is often the first face, and voice families encounter as they begin their journey with Colonial Poplin. Her warm, friendly demeanor helps put residents and their loved ones at ease from the very first interaction.

Michele Thomas, PTA
Director of Rehab Services at Colonial Poplin and Poplin Way
Michele is a long-standing member of the Colonial Poplin and Poplin Way team, bringing years of dedication, experience, and compassion to her role as Director of Rehabilitation Services. As a Physical Therapy Assistant, she brings valuable clinical knowledge and hands-on experience to help those under our care work toward their individual rehabilitation goals.
Michele collaborates with residents, clients, their families, and the interdisciplinary team to support personalized therapy plans focused on mobility, strength, and independence. Her passion for helping others and her deep understanding of rehabilitation have made a lasting impact on the many individuals she has supported throughout her career.

Jocelyn O’Reilly, MS-CMHC
Director of Social Services at Colonial Poplin
Jocelyn brings extensive experience in the senior living field to her role as Director of Social Services at Colonial Poplin. With a strong passion for advocacy and supporting the overall well-being of those we serve, she works closely with residents and families to help ensure their needs, preferences, and goals are understood and supported.
Jocelyn serves as a valuable resource for residents through emotional support, mental health services and referrals, community resource connections, and discharge planning assistance. Known for her compassionate approach and dedication to listening, she provides guidance and advocacy throughout each resident’s journey while helping promote a positive quality of life. Her commitment to building meaningful relationships and connecting residents and clients with the right resources reflects the mission and values of our community.

Business Office Manager of Colonial Poplin and Poplin Way
Shannon brings seasoned proficiency in senior living financial experience to her role as Business Office Manager for Colonial Poplin and Poplin Way. With a strong understanding of the administrative and financial operations within senior living, she supports residents, families, and responsible parties with billing, insurance, and financial-related questions.
Known for her professionalism, attention to detail, and approachable nature, Shannon helps ensure a seamless experience while supporting the daily operations of both communities and our commitment to exceptional resident care.

Aaron Deyermond, ADC, ALEC, CMDCP
Director of Community Life at Colonial Poplin and Poplin Way
Aaron oversees Life Enrichment services and their team members across both communities. A Board-Certified Activity Director/Assisted Living Enrichment Certified, and Certified Montessori Dementia Care Professional (CMDCP), Aaron is dedicated to enhancing the quality of life for every resident and/or client through meaningful, person-centered engagement.
Aaron began his career in senior living in Environmental Services before broadening his experience into recreation and nursing departments, giving him a well-rounded understanding of resident care and community operations. Together with his dedicated Life Enrichment team, he develops innovative programs, signature events, and meaningful opportunities that celebrate each resident's interests, abilities, and life story. He also assists with community outreach and marketing initiatives, helping strengthen relationships between Colonial Poplin, Poplin Way, and the surrounding community.

Director of Dining Services for Colonial Poplin and Poplin Way
Lynn brings extensive experience and dedication to her role as Director of Dining Services at our senior living community. She oversees the dining operations across both communities, ensuring our residents and clients receive quality meals that meet their nutritional needs and personal preferences.
With a strong attention to detail and commitment to consistency, Lynn works closely with residents, clients, staff, and the interdisciplinary team to support a successful dining program that enhances daily life within our communities.

Frank Ryan
Facilities Director
Frank oversees facilities operations for Colonial Poplin and Poplin Way, ensuring our communities remain safe, functional, and welcoming environments for residents, clients, families, and team members. Through his leadership of maintenance operations, he supports the daily upkeep, repairs, and improvements that allow our communities to operate smoothly.
With a strong commitment to safety, reliability, and effective problem-solving, Frank works behind the scenes to maintain the physical environment while responding to the evolving needs of both communities.
